If applicable, the following documents are required to be attached to your submission:
- McLean County Food Permit – A Health Permit is required if the applicant's business intends to sell any food (including ice).
- Agreement with Business/Property Owner – Businesses that plan to operate/park on private property must provide a letter of written permission from the business/property owner. The written permission must include:
- Applicant Business Information:
- The applicant's business Entity Name and DBA/Assumed Name; and
- Specific date(s) or date range the business is approved to operate/park on the property;
- Business/Property Owner(s) providing permission:
- The address of the property that will be used, and if applicable, the name of the business;
- Their full name, relation to the property (to show why they have the authority to provide permission), phone number, and email address; and
- Their signature.
Inspections
If approved, licenses will not be issued until inspections are approved. A City of Bloomington safety inspection may be required before approval can be given. An inspector will reach out to you if an inspection is required. If an inspection is not required, you will be notified of the final status of your application after the review of your application is complete.
Tax Requirements
The following form(s) are required to be submitted before completing the application: Note: All applicants must be in compliance with the City's Finance Department in order for a license to be considered.

- Food & Beverage Tax Registration Forms are used for businesses that have not operated in the City of Bloomington/Town of Normal before and need to register.
- Food & Beverage Tax Remittance Forms are used for businesses that are active and need to provide information about their taxable income.
- These forms should be completed each month, even if the business is reporting $0.
Additional Requirements
The following are required in order for applications to be considered and documentation is checked internally, so there is no submission requirement unless specifically requested.
- DBA/Assumed Name must be registered. If the Applicant will use a different name with the public than the name of the legal entity, the DBA/Assumed Name must be registered with the State of IL or McLean County.
- Entities must be in good standing with the State of IL.
