Please attach the following documents to your application upon submission:
- List of Owners, Partners, Officers, etc. and Managers of the Business
- Must include all of the following for each:
- Full Legal Name (as it appears on their Government-Issued ID);
- Title or Relation to Business;
- Personal Email Address;
- Date of Birth;
- Personal Address (including City, State, and Zip Code); and
- Percentage of Ownership/Stock.
- A clear photocopy of a Government Issued photo ID for each owner over 20% and Managers of the business
- To be accepted, your ID copy must be completely shown in the picture and not be blurry.
- If Applicant will use a different name with the public than the name of the legal entity, proof of the DBA/Assumed Name is registered with the State of Illinois or the County where the Establishment is located.
- Copy of State Liquor License AND Copy of Local Liquor License Where Establishment is Located
- Food/Beverage Tax Registration Form with the City of Bloomington Finance Department
- Certificate of Insurance (Not Policy Declaration Page) The Certificate must show the establishment has liquor liability insurance and must include the following:
- Applicant’s name as the insured (e.g. if the applicant is a corporation, the corporation’s name must be listed);
- The date, times, and address of the location where the liquor is being consumed;
- The dates of coverage and the coverage limits
- Certificate Holder must be listed as:
- City of Bloomington, 115 E. Washington St., Bloomington, IL 61701
- McLean County Food Permit (or food permit for the applicant's local municipality)
NOTE: IL Food Handler's Permit for the person(s) handling food for the event and BASSET Certificates for those selling/serving must be available upon request at the event.
