The following documents are required to be on file at all times during the license period, and must be included with your application submission:
- List of Owners, Partners, Officers, etc. and Managers of the Business
- Must include all of the following for each:
- Full Legal Name (as it appears on their Government-Issued ID);
- Title or Relation to Business;
- Personal Email Address;
- Date of Birth;
- Personal Address (including City, State, and Zip Code); and
- Percentage of Ownership/Stock.
- A clear photocopy of a Government Issued photo ID for each owner over 20% and Managers of the business
- To be accepted, your ID copy must be completely shown in the picture and not be blurry.
- McLean County Health Department Permit (Not Inspection Report)
- Certificate of Liability Insurance (Actual Certificate is Required)
- The Certificate is required to list all of the following:
- Legal Entity Name, DBA (Assumed Name), and Entity Address listed;
- Vehicle Make, Model, and License Plate # in the Description Box;
- The City of Bloomington, 115 E. Washington St., Bloomington, IL 61701, listed as the Certificate Holder (bottom left of Certificate); and
- Must have the City of Bloomington listed as an Additional Insured
- Additional Insured can be listed in the Description Box (near the bottom of the Certificate)
If applicable, the following must be included with your application submission:
Property/Business Owner Permission - Food Trucks that plan to operate/park on private property must provide a letter of written permission from the business/property owner. The written permission must include:
- Applicant Business Information:
- The applicant business Entity Name and DBA/Assumed Name; and
- Specific date(s) or date range the Food Truck is approved to operate/park on the property;
- Business/Property Owner(s) providing permission:
- The address of the property that will be used, and if applicable, the name of the business;
- Their full name, relation to the property (to show why they have the authority to provide permission), phone number, and email address; and
- Their signature.
The following form(s) are required to be submitted before completing the application: Note: All applicants must be in compliance with the City's Finance Department in order for a permit to be considered.

- Food & Beverage Tax Registration Forms are used for businesses who have not operated in the City of Bloomington/Town of Normal before and need to register.
- Food & Beverage Tax Remittance Forms are used for businesses who are active and need to provide information about their taxable income.
- These forms should be completed each month, even if the business is reporting $0.
Additional Requirements, if applicable.
- Entities must be in good standing with the State of IL. No documentation should be submitted from the business for this. The City Clerk Department will verify standing internally.
- Inspections - A City of Bloomington safety inspection may be required before approval can be given. An inspector will reach out to you if an inspection is required. If an inspection is not required, you will be notified of the final status of your application after the review of your application is complete.
