The following documents are required to be on file at all times, and should be included with your submission:
- List of Owners, Partners, Officers, etc. and Managers of the Business
- Must include all of the following for each:
- Full Legal Name (as it appears on their Government-Issued ID);
- Title or Relation to Business;
- Personal Email Address;
- Date of Birth;
- Personal Address (including City, State, and Zip Code); and
- Percentage of Ownership/Stock.
- A clear photocopy of a Government Issued photo ID for each owner over 20% and Managers of the business
- To be accepted, your ID copy must be completely shown in the picture and not be blurry.
- Premises Verification
- If the licensed location is not owned by the licensee, an active Lease Agreement is required to be on file at all times and must include:
- Name of Entity or Person(s) under Ownership;
- Establishment Address (including applicable suite or unit #s); and
- Term Dates.
- Certificate of Liability Insurance (not Policy Declarations)
- The Certificate must include:
- Applicant’s name as the insured (i.e., if the applicant is a corporation, the corporation’s name must be listed);
- DBA/Assumed Name, if applicable;
- Address of the establishment that holds the license;
- Dates of coverage(start and end);
- Coverage Limits
- Must hold a minimum amount of $1,000,000 General Liability
- The City of Bloomington listed as an Additional Insured(can be listed in the Description Box); and
- The City of Bloomington, 115 E. Washington St. Bloomington, IL 61701 listed as the Certificate Holder (bottom left of the Certificate).
- Surety/Penal Bond in the amount of $2,000
- The Bond must include:
- Entity/Individual Name;
- DBA/Assumed Name, if applicable;
- Establishment Address;
- Bond Number;
- Term Dates; and
- The City of Bloomington, 115 E. Washington St. Bloomington, IL 61701, should be listed as Obligee.
The following information is reviewed internally, so businesses should not submit the following unless specifically requested.
- DBA/Assumed Name must be registered – If the Applicant will use a different name with the public than the name of the legal entity, the DBA/Assumed Name must be registered with the State of IL or McLean County:

- Entities must be in good standing with the State of IL.
