The following documents are required to be on file at all times, and must be included with your submission:

• List of Owners, Partners, Officers, etc.
  • Must include all of the following for each:
    • Full Legal Name (as it appears on their Government-Issued ID);
    • Personal Email Address;
    • Date of Birth;
    • Personal Address (including City, State, and Zip Code); and
    • Percentage of Ownership/Stock. 

• Premises Verification
  •  If the licensed location is not owned by the licensee, an active Lease Agreement is required to be on file at all times and must include:
    • Name of Entity or Person(s) under Ownership;
    • Establishment Address (including applicable suite or unit #s); and
    • Term Dates.

• Certificate of Liability Insurance (not Policy Declarations)
  •  The Certificate must include all of the following:
    • Applicant’s name as the insured (i.e., if the applicant is a corporation, the corporation’s name must be listed);
    • DBA/Assumed Name, if applicable;
    • Address of the establishment that holds the license;
    • Dates of coverage (start and end);
    • Coverage Limits
      • Must hold a minimum amount of $500,000 per person and $1,000,000 in the aggregate per occurrence and property damage in a minimum amount of $1,000,000
    • The City of Bloomington listed as an Additional Insured (can be listed in the Description Box); and
    • The City of Bloomington, 115 E. Washington St. Bloomington, IL 61701 listed as the Certificate Holder (bottom left of the Certificate).

•  Surety/Penal Bond in the amount of $2,000
  • The Bond must include:
    • Entity/Individual Name;
    • DBA/Assumed Name, if applicable;
    • Establishment Address;
    • Bond Number;
    • Term Dates; and
    • The City of Bloomington, 115 E. Washington St. Bloomington, IL 61701, should be listed as Obligee.

The following information is reviewed internally, so businesses should not submit the following unless specifically requested.

• DBA/Assumed Name must be registered – If the Applicant will use a different name with the public than the name of the legal entity, the DBA/Assumed Name must be registered with the State of IL or McLean County:

NOTE: If already registered, this documentation is not required for submission and will be monitored internally. If not registered, please complete one of the following:

• State of IL Adoption of an Assumed Name (Corporations and LLCs)
• McLean County DBA/Assumed Name Registration  

• Entities must be in good standing with the State of IL.

All Commercial Vehicle Relocator applicants require a mandatory background investigation. The investigation will encompass state, federal, and local background checks and may include fingerprinting, if deemed necessary. The applicant shall be responsible for all investigation fees incurred. Applications will not proceed until this requirement is fulfilled.

Please be advised that the processing of background investigation reports may take up to one month, depending on the volume of results. Applicants are strongly encouraged to complete their portion of the process as soon as possible to prevent any delays in the application review. 

Within 2 business days of submission of the application, the applicant will receive an email from SRA Screening, the City’s background investigation company, with details on how to complete the required background investigation.

Note that additional fees may be assessed during the background investigation process. Additional fees could include fees from courts, government agencies, or other third parties that charge for access to records. If any additional fees apply, the applicant will receive an email with updated invoice information. Payment of any additional fees is required before a license can be issued.

Before license application review can begin, the applicant must pay the following Application and Background Investigation Fees:

Application Fee: $100
Investigation Fee (per person listed under ownership): $25*

*In addition to the background investigation fee listed above, the Applicant Entity will be responsible for any extra fees that may be assessed during the background investigation process. (Additional fees could include fees from courts, government agencies, or other third parties that charge for access to records). If any additional fees apply, the applicant will receive an email with invoice information. Note that payment of any additional fees is required before a license can be issued.

If approved, each entity is required to pay the following license fee:

Annual License Fee: $250

For an applicant's first annual license with the City Clerk Department, the license fee will be prorated based on the number of days remaining in the active term following approval.
